Being ambushed by ferocious Treasure Hoarders and killed because they failed to escape in time.
Encountering monsters such as Hilichurls and Slimes, accidentally getting swallowed by a Slime, or being dragged back as ingredients by man-eating Hilichurls and thus dying.
In short, the ways they lost people were bizarre and varied.
